The District Council have some information here about renewing it:

Renewing Your National Concessionary Bus Pass
If you currently hold a bus pass issued by Rochford District Council which expires on 31 March 2011, There is no need to contact the Council or complete a renewal application form, as your bus pass will automatically be renewed and sent to you in March 2011.
If, however, you have changed your name or address since your last bus pass was issued or you no longer require a bus pass, please contact Rochford District Council’s Concessionary Fares Department before 14 January 2011, so your records can be amended.
Contact us during office hours or email: transportation@rochford.go.uk

They have also the answers to a pretty comprehensive set of frequently asked questions. For example:

Q: I would like my new bus pass to have an updated photograph, what should I do?
A: If you provide a new photograph to the Council’s Concessionary Fares Department by the 10 January 2011, your new photograph will be used on your renewed bus pass.

Q: My partner relative/close friend has recently passed away, should I contact the Council and return their bus pass for cancellation?
A: Yes, you should return the bus pass to the Council as soon as possible for cancellation or the pass will automatically be renewed.

It’s good to see the bus pass scheme thriving – the Lib Dems had to battle away for years to get a bus pass scheme made available to all pensioners in our district.
